Three steps
How to get a consultation
No date is set before Dr Seffen has seen your photographs. This is not paperwork: it is what allows us to tell you, before you book a flight, whether surgery makes sense in your case.
You put your file together
A questionnaire and four photographs, on dry hair: front, profile, top and the back of the head, which is the donor area. Allow about ten minutes. The file is free and commits you to nothing.
Dr Seffen studies it and answers you himself
Within five to seven working days, given the volume of requests. You receive a primary diagnosis, the proposed strategy, an estimated graft count and a quote. He writes them himself, file by file.

Two appointments
Remotely, then at the clinic
Most of Dr Seffen's patients live abroad. The first opinion is therefore given on photographs, and the face-to-face consultation takes place in Tunis, the day before surgery.
The video call, before you book

The pre-operative consultation, in Tunis
The day before surgery, on the afternoon you arrive. This is the real examination, the one a photograph cannot replace. It can also take place on the morning of surgery if your flight allows nothing else.

The pre-operative consultation is compulsory for any patient who has not been seen in the two months before surgery. A scalp changes, and a quote based on last year's photographs is worth nothing.
What gets decided
Five questions we settle together
A hair transplant consultation is not a sales presentation. It is an examination, and it ends in five answers. Some are not pleasant to hear, and that is exactly why they are said before surgery rather than after.
1. What your donor area is worth
This is examined first, and it is the one thing that cannot be negotiated. The back and sides of the head hold a finite number of follicular units, particular to each person. That capital does not renew itself: what is harvested today will not be there in ten years. Densitometry measures what you actually have, and that figure governs everything else.
2. What you expect, said precisely
Restoring the hairline you had at twenty and thickening a sparse crown are not the same request, and they do not cost the same number of grafts. The more precisely you describe what you want, the more useful the answer. A photograph of yourself before the hair loss helps a great deal.
3. What is feasible, and what is not
Not everything is. A hairline set too low on a twenty-five-year-old whose hair loss is still progressing is a bad idea, even where it is technically possible: it ages badly, and it spends the capital that will be missing in fifteen years. Some patients are not candidates for surgery. You will be told so.
4. The graft count, and what it changes
The quote rests on an estimate, refined at the pre-operative consultation. The price is a package, not a price per graft: it does not change if the procedure takes two hundred grafts more than planned.
5. What comes before surgery, or alongside it
Active hair loss is not operated on without being treated. Depending on the case, Dr Seffen proposes medical treatment on its own, or alongside the transplant. A transplant placed into hair loss left to run gives a result that thins out around it.

Is the consultation paid for?
No. The primary diagnosis, the estimated quote and the optional video call are free. The pre-operative consultation in Tunis is included in the package.
Do I have to come to Tunis to get an opinion?
No. The first opinion is given on your photographs, and you receive it at home. You travel only if you decide to go ahead.
How long between the file and the answer?
Five to seven working days. Dr Seffen studies every file himself, which explains both the delay and what the answer is worth.
What if my file is more than two months old?
The pre-operative consultation becomes compulsory, the day before surgery. A scalp changes, and an old quote no longer describes the real state of the donor area.
Can I be turned down?
Yes, and it happens. An insufficient donor area, hair loss that is not stabilised, an expectation surgery cannot meet: in those cases Dr Seffen says so rather than operating.
